Documenting and sharing the lessons learned from mistakes ensures that the knowledge is retained and accessible for future reference. This can be done through post-mortem analyses, debriefing sessions and creating a repository of case studies.

For instance, after completing a project, review what went well and what didn't. Document the findings and share them with the team. This helps create a knowledge base that can be referred to when planning future projects.

Failing to document lessons learned can lead to repeated mistakes and lost knowledge, while capturing and sharing insights creates a valuable resource that enhances collective learning and improves future decision-making processes.
